Office Policies
Welcome to our clinic. We work hard to be able to offer a high quality of service and encourage you to be well informed of your health care coverage. Please review your policy for co-pays, co-insurance and deductibles. As a courtesy we will confirm your benefits with your insurance.
Auto Accident. Please open a personal injury protection (PIP) claim with your car insurance company. We will need your claim number, insurance company and adjuster information.
Worker's Compensation. Please file incident report with your employer. If we are the first provider, we will file form 827 for you. If you have seen another provider, we will need a referral.
